Usage of Tretinoin:
Isotretinoin or 13-cis retinoic acid is a very effective medication for the treatment of acne . It is a retinoid; this means it is derived from vitamin-A (retinoic acid). The liver naturally
makes small quantities of isotretinoin from vitamin-A, but the drug we prescribe is made synthetically.
Isotretinoin Description
1,Isotretinoin is the acid form of vitamin A and is also known as all-trans retinoic acid or ATRA. It is a drug commonly used to treat acne vulgaris and keratosis pilaris. It is available as a
cream or gel It is also used to treat acute promyelocytic leukemia, and is sold for this indication by Roche under the brand name Vesanoid. It is also available as a generic.
2 Its success in treating acute promyelocytic leukemia was a major breakthrough in the treatment of this type of leukemia.It works in APL because the majority of cases involve a chromosomal
translocation of chromosomes 15 and 17, which causes genetic fusion of the retinoic acid receptor gene to the promyelocytic leukemia gene. This fusion PML-RAR protein is responsible for
preventing immature myeloid cells from differentiating into more mature cells. This block in differentiation is thought to cause leukemia. ATRA acts on PML-RAR to lift this block, causing the
immature promyelocytes to differentiate to normal mature blood cells thus decreasing promyelocytes.
Isotretinoin Function
1.Isotretinoin's main indication is for the treatment of severe cystic acne vulgaris. Many dermatologists also support its use for treatment of lesser degrees of acne that prove resistant to
other treatments, or that produce physical or psychological scarring.
2.It is also somewhat effective for hidradenitis suppurativa and some cases of severe acne rosacea. It can also be used to help treat harlequin ichthyosis, lamellar ichthyosis and is used in
xeroderma pigmentosum cases to relieve keratoses. Isotretinoin has been used to treat the extremely rare condition fibrodysplasia ossificans progressiva. It is also used for treatment of
neuroblastoma, a form of nerve cancer.
3.Isotretinoin therapy has furthermore proven effective against genital warts in experimental use, but is rarely used due to potentially severe side effects. In a small-scale study, low dose oral
isotretinoin showed considerable efficacy;
4.Isotretinoin may represent an efficacious and safe alternative systemic form of therapy for RCA of the cervix. Not many studies have been conducted to further confirm its effectiveness. In most
countries this therapy is currently unapproved and only used if other therapies failed.
